Optimum management of asymptomatic moderate-severe degenerative mitral regurgitation: a role for T1 mapping in risk stratification?

چکیده
Methods In total, 24 patients (mean age 62 +/16 years) with degenerative moderate or severe asymptomatic MR with no class 1 indication for surgery were recruited from a dedicated valve clinic in a University Hospital and compared with gender matched controls. All subjects underwent cardiac MRI (1.5T Siemens Avanto) for assessment of LV volumes, deformation (SPAMM tagging), late gadolinium enhancement (LE) and T1-mapping using a modified look-locker inversion recovery sequence (MOLLI) before and 15 minutes post gadolinium (0.1 mmol/Kg) for myocardial extracellular volume (ECV). Global ECV was the average measure from basal and mid short axis slices excluding LGE indicative of infarcted myocardium.

Quantification of left ventricular interstitial fibrosis in asymptomatic chronic primary degenerative mitral regurgitation.
BACKGROUND The optimum timing of surgery in asymptomatic patients with chronic severe primary degenerative mitral regurgitation (MR) remains controversial, and further markers are needed to improve decision-making. There are limited data that wall stress is increased in MR and may result in ventricular fibrosis. We investigated the hypothesis that chronic volume overload in MR is a stimulus for...

The role of exercise testing and stress imaging in the management of patients with valvular heart disease (VHD) is reviewed in this article. The American College of Cardiology/American Heart Association and the European Society of Cardiology/European Association of Cardiothoracic Surgery have recently put emphasis on the role of exercise testing to clarify symptom status and the use of stress i...
متن کاملHigh-risk Coronary Artery Bypass Grafting and Mitral Valve Replacement in a HIV Positive Patient
Certain subsets of high-risk mitral valve patients are not suitable candidates for transcatheter therapies. The objective of this report is to present a young patient with combined mitral valve and coronary artery disease to illustrate these challenges. In this report, we present a 47-year-old man with longstanding HIV infection who was referred with severe mitral regurgitation (MR) and profound...
متن کاملErratum: Surgical Timing of Degenerative Mitral Regurgitation: What to Consider
Severe primary mitral regurgitation (MR) is a progressive condition which engenders significant mortality and morbidity if left untreated. The optimal timing of surgery in patients with MR of degenerative origin continues to be debated, especially for those who are asymptomatic. Apart from symptoms, current authoritative guidelines recommend intervention when there is incipient left ventricular...
متن کاملOutcome of watchful waiting in asymptomatic severe mitral regurgitation.
BACKGROUND The management of asymptomatic severe mitral regurgitation remains controversial. The aim of this study was to evaluate the outcome of a watchful waiting strategy in which patients are referred to surgery when symptoms occur or when asymptomatic patients develop left ventricular (LV) enlargement, LV dysfunction, pulmonary hypertension, or recurrent atrial fibrillation. METHODS AND ...
